Songs do not get uploaded [URGENT]

I have installed a fresh and unmodified copy on 2 servers now and the upload functionality has stopped working.

I am able to go to the upload page, chose a file, fill in all the details, hit the upload button. It will actually have a message that says "Upload was successful" appear at the top.

Then under "My Audio" I see the file I just uploaded, but it states it is 0:00 in length, and when I go to the page, nothing plays. There are also no song files in my /uploads/ or /adata/ folders at all.

The weird thing is that the upload functionality was working until a few hours ago, now it isn't.

I hope we can get this resolved urgently.

I posted the url earlier, but I don't know if it went through or not. So I'm posting again:

The URL is http://132.216.109.133/index.php of our customized version, (which uses the default upload.php and upload.tpl files) and a completely stock version here that has the same upload problem without any modified files on a different server here: http://74.50.8.180/~ilevi3/audio/

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.
